Before treatment:
Manuel was delivered by cesarean section during the 38th week of pregnancy. He suffered from breathing difficulty after birth. With the assistance of a ventilator, the patient's breathing condition improved and he was discharged from the hospital five days later. Manuel became blind when he was five months old. He also had frequent epileptic seizures and most were grand mal seizures. He was prescribed Trileptal to control the seizures, but the medication was ineffective. The medications were changed to "Keppra", Acido Valproico and "ACTH", but the seizures were still out of control.  At one and a half years old, the Keppra and Frisium, were better able to control the epileptic seizures.  He was diagnosed with West syndrome, optical nerve atrophy and callosum hypoplasia when he was three years old. He was unable to turn himself over, and could hardly maintain a sitting position. He was also unable to talk. After he was treated in our hospital during September of 2014, his movements and speech had improved. Now he can turn himself over normally, and can maintain a sitting position, but he cannot sit, climb, stand up or walk without assistance. He can speak a few words, such as papa or mama. Currently he is taking “Keppra” 300mg, (twice a day), “Topiramate” 25mg(once a day). The epileptic seizures occur every two weeks and he will have spasms in his upper limbs lasting for about 30 seconds. Currently, he has some family staying with him at our hospital during the continued treatment.

Admission PE:
Bp: 88/60mmHg; Hr: 88/min. Br: 20/min. Height: 111cm, weight: 28Kg. The development was slower than normal. The nutrition was normal. There are no yellow stains on the skin. The thorax was symmetrical. The respiratory sounds in both lungs were clear, with no obvious dry or moist rales. The rhythm of the heartbeat was irregular. The heart sounds were strong. There was no obvious murmur in the valves. The abdomen was flat and soft, without pressing pain or rebound tenderness. The liver and spleen were normal and there was no swelling in the lower limbs.

Nervous System Examination:
Manuel Alejandro was alert. The patient has a speech disorder and could only pronounce words like "Papa and Mama." He had memory, calculation and orientation difficulties during the examination. His comprehension was poor. Both eyes had visual impairment (he couldn't cooperate with the vision examination).He was able to respond to strong light in darkness and the diameter of both pupils was 2.5mms. The responses were slow to light stimuli. The forehead wrinkle pattern was symmetrical. Bilateral nasolabial sulcus was equal in depth. The neck moved normally and he had good control ability of the head while in the sitting position. He could turn over by himself normally. The muscle strength of the waist and back was comparably poor. He was able to maintain a sitting position when he had support from both hands to keep balance. The grasping ability of both hands was poor, butall four limbs could move normally. He couldn't cooperate with the examination of the muscle strength of all four limbs. The muscle tone of all four limbs was nearly normal. The abdominal reflex was normal. The bilateral patellar tendon reflex was active and the bilateral pathological sign was negative.  He was unable to cooperate with the examination of sensation or coordinated movements.
 
Treatment:
The patient was diagnosed with West syndrome and optical nerve atrophy. He received four nerve stem cells injections and four mesenchymal stem cells injections  to activate the repair and regeneration of the nerves in order to supplement the number of existing nerve cells in the brain, activate stems cells in the body, strengthen the immune system, improve the circulation, and control the epileptic seizures. The patient was also given physical rehabilitation training.

Post-treatment:
After 21 days of treatment, the overall functioning has improved, and his vocabulary has increased. The flexibility of all four limbs has improved and the stomach muscles have gotten stronger and as a result, he can do sit-ups with assistants from others. The eyes are more sensitive to light and a better protected from radiation.  The eyes are able to follow large objects.  After the treatment was completed, Manuel’s overall physical condition had improved and he was able to be discharged from our hospital.

Before treatment:
The patient was born at 38th week of pregnancy.The weight: 2.3Kg. The patient suffered from breathing difficulty after birth. With assistance of ventilator, the patient's breathing condition improved and he was discharged from the hospital. The family member found the patient had vision problem when the patient was 1 month old. He was sent to hospital, but he didn't received specific diagnosis. The patient still had vision disorders when he was 5 months old. He had frequent epileptic seizure and most of them were grand mal. He received Trileptal from a local hospital for anti-epileptic. But the epileptic seizure was still out of control. He changed medication to "Keppra" and "Frisium", the epilepsy was then brought under control. He was diagnosed with West syndrome. Before the treatment, the patient was unable to sit-up, but still he could maintain sitting position. He was unable to crawl or walk. He had speech disorders and only could say "A". He took "Keppra" 600mg BID, Frisium2.5mg BID, 5 mg 1 time before sleep. The last epileptic seizure was in February 2014. This was a grand mal. 
   
From the onset of disease, the patient's spirit was good. His intelligence and movement was delayed. His diet was normal. The sleep quality was normal. He suffered from gatism. There was no genetic disease.

Admission PE: 
Bp: 105/65mmHg; Hr: 77/min. Br: 24/min. Height: 108cm, weight: 22Kg. The development was slower than normal. The nutrition was normal. The skin and mucosa was intact, with no yellow stains or petechia. The thorax was symmetrically. The respiratory sounds in both lungs were clear, with no obvious dry or moist rales. The rhythm of his heartbeat was irregular. The heart sounds was strong. There was no obvious murmur in the valves. The abdomen was flat. The liver or spleen under the ribs was not enlarged.

Nervous System Examination: 
Manuel Alejandro was alert. The patient had speech disorder, only could pronounce the alphabet "A". He was unable to cooperate with the examination of memory, calculation or orientation. He had poor comprehension. Both eyes had visual impairment (he couldn't cooperate with the examination of vision). Both pupils were equal in size and round, the diameter was 3.5mms, both eyeballs had slowly response to light stimulus. The forehead wrinkle pattern was symmetrical. Bilateral nasolabial sulcus was equal in depth. The neck moved normally. He had poor control ability of head in sitting position. The control ability was poor when he raise head in supine position. He could turn over by himself occasionally. The muscle strength of waist and back was poor. He had difficulty to maintain sitting position by himself. The grasp ability of both hands was poor. There was activities in his four limbs. He couldn't cooperate with the examination of muscle strength of four limbs. The muscle tone of four limbs was near-normal. The abdominal reflex was normal. Bilateral patellar tendon reflex was active. Bilateral achilles tendon reflex disappeared. Right side Babinski sign was weak positive. He was unable to cooperate with the examination of sensation or coordinate examination.

Treatment:
We initially gave Manuel Alejandro a complete examination and he was diagnosed with West syndrome. The patient received treatment for nerve regeneration and to activate stem cells in vivo. He received treatment to improve the blood circulation in order to increase the blood supply to the damaged neurons, enhanced immunity and to nourish them. We also adjusted the patient's diet and improved the nutrition. This accompanied with daily physical rehabilitation.

Post-treatment: 
The patient's height: 108cm, weight: 24 Kg. The chewing ability is better than before.  He can chew and eat a handful of snacks. The muscle strength in neck, back and abdomen have improved. He can turn over and sit-up by himself. He can maintain sitting position for a longer time. He has better control of trunk in sitting position. Without any support, the patient does not fall frequently to lateral side in sitting position. He can maintain kneel position. Both hands has grasp action. Right hand can grasp toy. Left hand is slightly poor than right hand. He can hold feeding bottle and drink milk.

Buenas tardes, adjunto los examenes de laboratorio de Manuel Alejandro Escalona, disculpe que no se le habían enviando con anterioridad porque no estábamos en el país.

Manuel actualmente esta diciendo palabras como: mama y papa, ha aumentado de tamaño y ya le salieron sus dos dientes que había mudado.
Por otro lado, esta presentando unos pequeños espasmos a la hora de dormir, su neurologo le aumentó el clobazam a 20mg. Diario.

Buenas tardes, un cordial saludo. adjunto los resultados de analisis de laboratorio y de orina de Manuel Alejandro Escalona Iwasaki. Ya terminsus medicinas, esta un poco mejor, se mantiene mas tiempo sentado y trata de decir palabras.
