Before treatment:
Ernesto Fragoso Cavaco had pain in both ankles 15 years ago. He went to a local hospital and diagnosed as arthritis. The doctor considered it and related it to long-time weight lifting, so he was not given any treatment. His experienced mre pain at the ankles 8 months ago and his lower limbs were dropsical. He took diclofenac sodium 100mg q12h to reduce 50% pain. He wanted a better treatment so he came to our hospital.

His spirit was good. His diet, sleeping, urination and defecation were normal. His weight was normal. He had hypertension for 4 years and took olmesartan 40mg every day.

Admission PE:
Bp: 169/91mmHg; Hr: 66/min. The respiratory sound in both lungs was clear, with no dry or moist rales. The heart sound was strong. The cardiac rhythm was regular, with no obvious murmur in the valves. The abdomen was soft with no masses and shifting dullness was negative. From his ankles to knees, there was 10cm depth of pitting edema without pressing pain. His legs were painful when they bore load and the flexibility of the joints were limited. The pulse of dorsalis pedis was normal and the skin temperature was normal.

Nervous System Examination:
Ernesto Fragoso Cavaco was alert and spoke fluently. His calculation, memory, and orientation were normal. His eyeballs move normally and both pupils were equal in size and round, the diameter was 3.0mms. Both eyeballs reacted normally to light stimulus and had no nystagmus. The forehead wrinkle pattern and nasolabial groove were symmetrical. His tongue was centred in the oral cavity. The muscle tone of his four limbs was normal and the muscle strength was at level 5. The tendon reflex of upper limbs was normal and the tendon reflex of lower limbs and abdominal reflex could not be elicited. The coordination movement was normal and the pathological sign was normal and meningeal irritation sign was all negative.

Treatment:
We initially gave Ernesto Fragoso Cavaco a complete examination and he received treatment to nourish neurons, relieve pain and prevent inflammation. During his stay in the hospital, he had blood pressure was high multiple times, 140-190/90-100mmHg. He took oral medicine to stabilize the blood pressure. 

Post-treatment:
After the treatment, patient’s blood pressure has been well controlled, 130-140/80-95mmHg. The supraventricular bearing premature was less. He can feel that his pain at the joints has been alleviated and joints ae not swollen.
